Post-Hospital Care at Home in Fairfax, Virginia
Same-day caregivers who make the move from hospital to home feel like a return, not a risk.
Creating Meaningful Days for Your Loved One at Home
Coming Home Should Feel Like Coming Home
Not like a continuation of the hospital by other means. Not like a transfer of responsibility from one set of strangers to another. Not like a folder of discharge instructions sitting on the kitchen counter that nobody fully understood.
Coming home should feel like relief. Like the familiar smell of your own house, the right pillow, the window you have looked out of for thirty years. It should feel like the hard part is behind you and the getting-better part has begun.
For families in Fairfax, Arlington, Alexandria, Springfield, and across Northern Virginia, post-hospital care through Thrive at Home is what makes that possible.
Our team is available 24 hours a day, 7 days a week. Call (703) 383-9300 to start the conversation.
The Window That Matters Most
One in five Medicare patients is readmitted to the hospital within 30 days of discharge, a rate documented by the Centers for Medicare and Medicaid Services. In most cases the reasons are preventable. A missed medication. A fall on the way to the bathroom. Poor nutrition during a period when the body needs everything it can get.
The first days and weeks at home are not a recovery plateau. They are the most critical stretch of the entire recovery. Most discharge plans assume that someone at home will handle what the hospital was handling. Thrive at Home is what closes that gap.

Same Day. Because Discharge Does Not Wait.
We know how discharge works. The timeline compresses. The folder appears. Suddenly 48 hours feels like no time at all.
Thrive at Home can arrange post-hospital care same day in most cases. Call us, a Registered Nurse conducts a complimentary on-site assessment, and care begins. Your loved one comes home to a prepared, attentive caregiver, a care plan, and an agency standing behind every hour of support.

This letter is to express my gratitude for the wonderful care and support your team provided to my Husband SK while under your care. Special thanks to S who was our angel in the form of our caregiver. I can’t say enough good things about S that could describe what relief she brought to us. She was kind, knowledgeable, happy and best of all she knew what we needed which changed daily.Everyone who was sent to care for SK did a commendable job as did the office staff. Muriel who took so many of my calls was always patient and understanding. What your team did for us will always be remembered and appreciated and I hope they know how important the job they do really is.Gratefully
“My mother has been a client with Thrive at Home for the last six years. The primary daytime caregiver is awesome, knows mom well and has dealt kindly with her changing personal and medical needs. The other caregivers who come, when the primary can not, are also great. I have been comfortable in leaving mom in their capable and experienced hands, to include some overnight stays. The office personnel are easy to work with, very helpful and work with scheduling needs. I would highly recommend Thrive at Home if you need part-time or full-time care for your loved one”
Isata is the perfect match for my mother. She is caring and very professional. My mom loves her

I appreciate the care givers positive attitude and going with my father to PT so they observe what he is doing in order to practice it when he is in his apartment
I had Carol for 5 days, 4 hours a day. She was competent, caring and a pleasure to help me with my every need
There are no more scary moments having to decide who will take care of my parents and who to trust. Thrive at Home has provided not only exceptional care for my parents, but also support and comfort for me!

Post-Hospital Support FAQs
Every Thrive at Home caregiver is a thoroughly screened employee with a minimum of two years of experience. Criminal background checks, driving record checks, skills testing, professional references, and tuberculosis testing are all part of the process. Our owner reviews every application personally.
It depends on the nature of the hospitalization and how the individual recovers. Some families need intensive support for two to three weeks. Others continue at a reduced level for several months.
Non-medical recovery care is not covered by Medicare. Long-term care insurance frequently covers transitional care. Some Medicare Advantage plans include home support benefits.
Home health care is medical care ordered by a physician and covered by Medicare for a limited period. Our post-hospital care is non-medical daily living support, present as often as needed to fill the gap between clinical visits. The two services work together.
Same day in most cases. Call as soon as you know discharge is approaching, even before the exact date is confirmed, and our team will begin the process immediately.
