Marie Woodard, RN, Thrive at Home Administrator and a member of the Fairfax County Long Term Care Coordinating Medical and Social Services Committee, will be holding two 90 minute presentations in September to walk you through how to develop and update a personal health care record.
Questions to ask yourself – when you go to your physician, the emergency room or a specialists are you be able to:

1.       List your medications quickly?

2.       List your past hospitalizations and surgeries?

3.       List your allergies?

4.       Provide the date of your last tetanus shot?

5.       Provide your emergency contact name and number?

This information is critical to your ability to receive effective medical treatment, especially in an emergency.

The Fairfax County Long Term Care Coordinating Committee wants to ensure county residents are aware of the importance of creating and maintaining a personal health care record.
